Immunization Information

ATTENTION PARENTS/GUARDIANS:
The State of Georgia requires that all students have an up-to-date Georgia Certificate of Immunization on file.  Therefore, please ensure your child's immunizations are up to date for the 2023-2024 school year.  Students whose immunizations are NOT up to date (by an annually established deadline) will not be able to attend school until the immunization requirement is complete.

The required Georgia Certificate of Immunization (Form 3231) can be obtained from your private physician or from the local health department.
